What is OTC Drug Monograph

OTC drugs are defined as drugs that are safe and effective for use by the general public without seeking treatment by a health professional.

There are over 300,000 marketed OTC drug products of over 80 therapeutic classes of drugs, for example, analgesics or antacids, instead of individual drug products. For each category, an OTC drug monograph is developed and published in the Federal Register.

An OTC monograph is a “rule book” for each therapeutic category establishing conditions, such as active ingredients, uses (indications), doses, labeling, and testing, under which an OTC drug is generally recognized as safe and effective (GRASE) and can be marketed without a New Drug Application and FDA pre-market approval.

Once a final monograph is implemented, companies can make and market an OTC product without the need for FDA pre-approval. These monographs define the safety, effectiveness, and labeling of all marketing OTC active ingredients.

New products that conform to a final monograph may be marketed without further FDA review. Those that do not conform must be reviewed by the New Drug Application process. A drug company may also petition to change a final monograph to include additional ingredients or to modify labeling.

Over-the-counter medicine is also known as OTC or nonprescription medicine. All these terms refer to medicine that you can buy without a prescription. They are safe and effective when you follow the directions on the label and as directed by your health care professional. Nonprescription drug products marketed under the OTC Drug Review are referred to as OTC monograph drugs.

How to Import OTC Drugs into the US

An OTC Drug Product is a product marketed for use by the consumer without the intervention of a health care professional to obtain the product. In addition to marketing under an approved new drug application or abbreviated new drug application, OTC drug products can be marketed in compliance with an OTC monograph regulation. OTC monographs represent regulatory standards for the marketing of non-prescription drug products without an approved new drug application. An OTC drug can be imported into the United States under an approved new drug application, an approved abbreviated new drug application, or in compliance with an OTC monograph.

  • The OTC monographs and ingredients covered by the monographs are provided by the following website: Status of OTC Rulemakings
  • The Small Business Assistancewebsite provides the requirements for the approval of a new drug application or an abbreviated new drug application

Both websites provide contact information for inquiries.

Currently there are still several categories of OTC drug products that are not subject to final regulations. Proposed monographs at the OTC website cited above provide guidance for the marketing of products not yet subject to a final rule.
